Prolonged Latency in Previable PPROM in Twin Pregnancies: A Case Series

Previable Preterm Premature Rupture of Membranes (PPROM) before 24 weeks of gestation in dichorionic diamniotic (DCDA) twin pregnancies is a rare and challenging obstetric complication. Three cases are presented in this case series, all involving prolonged latency periods of 74, 98, and 158 days following membrane rupture. The management strategy employed was expectant, utilizing antibiotic prophylaxis and close monitoring to prevent infection. Neonatal outcomes varied, with pulmonary hypoplasia and respiratory distress observed in some cases. One twin from Case 2 died due to sepsis, while the remaining twins in all three instances survived after extended neonatal intensive care. This series highlights the potential for extended latency and favorable outcomes in select cases, though neonatal morbidity remains a significant risk, underscoring the need for individualized counseling and vigilant care.

Minimising Carbon Footprint in Anaesthesia Practice

Carbon footprint refers to the total amount of greenhouse gas emitted in the atmosphere by an individual or by an organization causing global warming. There are various causes of greenhouse gas emissions and anaesthetic gases are one amongst them. Greenhouse gases warm the earth by absorbing infrared energy and slowing the rate at which the energy escapes into space. Each greenhouse gas has two important properties. One is the amount of infrared energy that a gas can absorb and the other is the lifetime of that gas in the atmosphere. Anaesthesia as a speciality contributes to carbon footprinting in three ways: direct emission, energy use, and operating room wastes and supplies. Direct emission of Waste Anaesthetic Gases (WAG) from anaesthesia workstations can either be scavenged and thrown out into the environment causing a green greenhouse gas effect or can pollute the operating room due to poor scavenging. Various techniques such as reducing direct emissions, energy use optimisation, and waste management have been tried in minimising carbon footprint in anaesthesia practice but providing safety to the patient is considered of utmost importance.

Occult Pneumomediastinum - An Atypical Presentation of Chest Discomfort in a Patient with Depression

Pneumomediastinum (mediastinal emphysema) is an uncommon condition characterized by the accumulation of air or gas in the mediastinum. Here is a case of a 16-year-old female known to have depression who presented to the emergency department with complaints of shortness of breath, restlessness, chest discomfort, and hoarseness of voice for 2 days. She was initially diagnosed with panic attack, and later on clinical examination, surgical emphysema over the right supraclavicular area was noticed. Chest X-ray was found to be normal, and further imaging with high-resolution computed tomography (HRCT) of the thorax showed pneumomediastinum. In this report, the clinical presentations, radiological features, and management of pneumomediastinum will be discussed.

The Clinical Pregnancy and Live Birth Following Transfer of One Arrested Embryo: A Case Report

Background: One of the problems in vitro fertilization (IVF) treatment for infertility is the high frequency of embryo developmental arrest in the preimplantation stages. Arrested embryos were not selected for transfer and were usually discarded. Case report: We present a case of clinical pregnancy and live birth following IVF treatment and transfer of one arrested embryo. A 31-year-old woman with unexplained infertility underwent IVF treatment. Using the IVF procedure, 7 embryos were produced which were frozen on day 3. In order to embryo transfer in the blastocyst stage, two embryos were thawed and cultured for 2 days. After thawing, one of them was not suitable for transfer and another embryo was arrested at the 10-12 cell stage. Discussion: The Clinical pregnancy and live birth happened after the transfer of an arrested embryo on day 5.Conclusion: This case showed that arrested embryos may resume growth after the transfer to the uterus and result in a successful pregnancy and live birth.

Analysis and Comparison of Social Media Applications Using Forensic Software on Mobile Devices

With the integration of mobile systems into daily life, social media applications used especially on Android and iOS platforms contain a significant amount of sensitive information. Social media applications on mobile systems have huge personal and sensitive content. Therefore, it is important to design effective techniques for forensic analysis of social media applications and to detect personal data. In this research, three different paid mobile forensic software and 4 different brands and models of smartphones with different operating systems were used and analyzed. The study shows that private messages, e-mails, time information, shared data, location and time information, and other personal data can be obtained by a forensic expert who performs an examination, and it is seen that one software can access the deleted data, but another software cannot access it. In proportion to the technology used in today’s world, mobile forensics incidents are increasing day by day, and a competitive environment is created among the software used to illuminate these incidents. With this competition, software companies dealing with forensic informatics are trying to obtain different data to illuminate forensic events that may occur due to the active use of social media accounts with the developing technology, and software that does not meet the needs in the face of this situation remains in the background. The criminal elements in the investigation areas of mobile forensics differ daily, and the scope of crimes in the virtual environment is expanding with the developing technology. Therefore, mobile forensic analysis applications should be successful in social media applications other than standard data.

Male Linear Anthropometrics of Selected Nigerian Ethnicities: A Cross - Sectional Analysis

Introduction: This study aims at evaluating selected linear anthropometrics of three Nigerian ethnic groups to provide baseline data for the creation of 3D Negroid anatomic models.Methods: The research design was a cross-sectional design. The sampling technique was multistage proportionate random sampling. The places of study were Imo, Oyo, and Kano States of Nigeria. The study lasted for one (1) year. Random selection of 1500 adult males from three major tribes (500 Igbo, 500 Yoruba, and 500 Hausa between the ages of 18 and 40 years). Tukey’s Post Hoc test of multiple comparisons was carried out to determine the specific ethnic groups that differ in specific anthropometric parameters.Results: The differences in standing height, arm length, and thigh length across the Hausa, Igbo, and Yoruba ethnic groups are statistically significant (p < 0.05). Conclusion: The study concluded that the Igbo and Yoruba groups had higher standing heights compared to the Hausa group. Arm length was longer in the Igbo and Yoruba groups compared to the Hausa group. However, thigh length was greater in the Hausa group compared to both the Igbo and Yoruba groups, while the Hausa group had longer thigh lengths than both the Igbo and Yoruba groups. The Igbo group displayed the largest arm span, whereas the Hausa group had the widest shoulder breadth. However, the Hausa group had a lower bi-iliac breadth in comparison to the other two ethnic groups.

Calorie restriction has been shown to slow the aging process in numerous organisms including primates. Caloric excess states, such as type 2 diabetes, are associated with accelerated aging and the incidence and severity of chronic diseases. The nutrient-sensing pathways and intestinal microbiome are important systems that affect aging and chronic disease development. This manuscript reviews the various pathways involved with aging and chronic disease development and examines the pharmacological manipulation of these systems which appear to slow aging and the chronic diseases of aging in experimental model organisms and collaborating human data when available. Finally, the abundance of experimental and human data suggesting the newer diabetic medications, the sodium-glucose transport inhibitors, are potent anti-aging agents is provided.

Understanding and Managing Caesarean Scar Ectopic Pregnancy: A Retrospective Analysis of Risk Factors, Strategies, and Outcomes

Objective: Caesarean Scar Ectopic Pregnancies (CSEP) pose distinct diagnostic and management challenges. This study investigates the risk factors, treatment options, and outcomes for CSEP cases treated at Prince Sultan Medical Military City, Riyadh.Methods: Over a two-year period, medical records of 28 patients with CSEP were reviewed, collecting data on patient demographics, obstetric history, and treatment outcomes. This analysis aimed to identify influential risk factors and assess the effectiveness of various management strategies.Results: Treatment success was achieved in 85.7% of cases, with Methotrexate (MTX) proving effective in over half of the cases. The findings suggest significant risk factors and best practices for managing this rare condition.Conclusion: Early detection, risk factor awareness, and tailored treatment strategies are key to optimizing outcomes in patients with CSEP.

Application of Multi-criteria Decision-making Methods for Forensic Analysis of Mechanical Parts in Vehicle Accidents using q-Rung Orthopair Fuzzy Numbers

This study explores the application of a Multi-Criteria Decision-Making (MCDM) approach based on q-rung orthopair fuzzy numbers to identify the origin of mechanical parts found at vehicle accident scenes. The primary objective is to determine the most likely vehicle to which these parts belong by evaluating key criteria such as compatibility, damage level, serial number matching, and color compatibility. Q-rung orthopair fuzzy numbers offer an advanced method to handle the inherent uncertainty and vagueness associated with forensic evaluations, particularly in scenarios where data is incomplete or imprecise. The proposed methodology involves defining the criteria, assigning membership and non-membership degrees using q-rung orthopair fuzzy sets, and applying an aggregation process to effectively rank alternatives. This approach facilitates flexible decision-making by accommodating different levels of confidence and uncertainty, making it particularly suitable for forensic applications where evidence is often ambiguous. The findings demonstrate that integrating q-rung orthopair fuzzy numbers into the MCDM framework significantly enhances the accuracy and reliability of identifying vehicle components involved in accidents. The proposed methodology provides a systematic tool to support forensic investigations, aiding in the determination of liability and contributing to more robust outcomes in mechanical evidence analysis.

Children who experience Adverse Childhood Experiences (ACEs)—including trauma, family instability, or significant loss—are at increased risk for enduring mental, emotional, and physical challenges, including conditions such as depression, anxiety, and chronic health issues. Without effective support, these adverse experiences can have lasting effects on a child’s well-being, often persisting into adulthood and impacting mental, emotional, and physical health outcomes over the long term. Storytelling is a powerful method to support resilience in these children by enabling them to process emotions, make sense of complex experiences, and view themselves as active agents in their lives. By framing their experiences within personal narratives, children can create coherent stories that aid emotional regulation and support mental well-being. Through storytelling, children can frame their struggles as part of a larger, purposeful journey, fostering a sense of agency and self-worth. Techniques such as narrative therapy, story circles, and resilience-themed books-exemplified by The Adventures of Gabriel—help children explore their experiences, visualise positive outcomes, and strengthen social bonds. Moreover, storytelling is an accessible tool that aids in perspective—taking, allowing children to relate their challenges to those of characters, which reinforces their ability to overcome adversity and navigate life’s complexities with strength and optimism.

Comparing Immunity Elicited by Feedback and Titered Viral Inoculation against PEDV in Swine

Porcine Epidemic Diarrhea Virus (PEDV) can infect pigs of any age, but the disease severity varies significantly, particularly affecting neonatal piglets due to their immature immune system. Various vaccination strategies have been questioned for their efficacy, especially since outbreaks have occurred even on vaccinated farms. Recent suggestions indicate that exposure to the virus may enhance the effectiveness of inactivated vaccines, highlighting the potential benefits of using attenuated viruses to generate immunity in sows without prior exposure. This study aimed to evaluate the humoral and cytokine responses in pregnant sows and their piglets after inoculation of affected piglet intestinal contents and a virus isolated. We measured immune parameters such as IL-12, IL-22, IgG, and IgA, as well as neutralizing antibodies in serum, colostrum, and milk. Notably, higher titers of neutralizing antibodies were found in sows immunized with the viral inoculum, while IL-12 and IL-22 levels showed no significant differences. Additionally, we assessed productive parameters like total piglets born, weaning mortality, average birth weight, and stillborn rates. The results indicated that sows treated with affected piglet intestinal contents had higher mortality (48.31%) and stillborn rates (20.96%) compared to those receiving the isolated virus (30.02% and 10.44%, respectively). These findings suggest that using an isolated virus can offer a safe, long-lasting, and specific immune response, underscoring the importance of thorough analysis of both systemic and mucosal immune responses against PEDV.

The idea behind this movement, known as human enhancement, has evolved over time from simple means of training to the improvement or augmentation of physical ability through recent developments in fields such as pharmaceuticals and implantable devices that can enhance natural abilities. They span from pharmaceutical agents to wearable technologies used for sophisticated self-experimentation, which offer promise but also present ethical, health, and societal risks Performance-Enhancing Drugs are used to enhance the performance of humans in one way or another as a basic idea. The categories of Performance Enhancing Drugs (PEDs) included are Stimulants: Stimulant drugs, including amphetamines and caffeine (in the form of coffee) have been used for thousands of endeavours and physical stamina for decades. There have been many sources of controversy in the sporting world due to use or misuse and side-effectual restrictions. The release of neurotransmitters such as dopamine and allowing muscles to grow big, and fast. Nevertheless, the consumption of anabolic steroids is strictly controlled because it has side effects and complications such as liver damage, hormonal imbalances, and greater aggression. Erythropoietin EPO is a hormone that produces red blood cells, and increased oxygen delivery to muscle. EPO Athletes and even more notably endurance athletes (such as cyclists) have a storied history of abusing this performance-enhancing agent to build stamina. However, the misuse of this drug can be very hazardous, as it causes complications such as blood clotting and heart issues.

Treatment Options for Congenital Dyserythropoietic Anemias (CDAs): Advances in Bone Marrow Transplantation, Gene Therapy, and Targeted Therapies

Congenital Dyserythropoietic Anaemia (CDA) is a rare genetic disorder that affects the maturation of red blood cells. The disorder is classified into different types, with a prevalence ranging from 1 in 100,000 to 1 in 1,000,000 individuals. Treatment strategies are designed with the primary focus on symptom management, the prevention and treatment of complications, and the underlying disease pathophysiology. The advent of bone marrow transplantation, gene therapy, and targeted therapies has considerably expanded the scope for therapeutic intervention in CDAs. Supportive care, including blood transfusions and iron chelation therapy, has demonstrated efficacy in managing iron overload and improving overall survival rates. The potential of gene therapy, targeted therapies, and hematopoietic growth factors in the treatment of CDA is currently being investigated. Further research and clinical trials are required to develop more effective and personalized therapeutic interventions.

Physiotherapy Undergraduate Students’ Perception About Clinical Education; A Qualitative Study

Background: Clinical education is an important component of physiotherapy education programs. It provides insights to the students in the fields of physiotherapy practice, develops leadership, and enhances their clinical skills in patient assessment, examination, diagnosis, treatment, planning, and intervention selection. The dimension of physiotherapy education is in the developing phase in Nepal. For further growth, the experience and the perception of the students are very important. Through this, the facilitators and barriers in physiotherapy clinical education need to be recognized. Therefore, this study aimed to explore the facilitators and barriers in physiotherapy clinical education in Nepal from a student’s perspective.Objectives: The main objective of this study was to identify the perception of physiotherapy undergraduate students about their clinical education and to find out the facilitators and barriers in clinical education.Methods: A qualitative research using focused group discussion was conducted. Ethical considerations were taken from respective departments and IRC approval was taken. BPT 3rd year, 4th year, and intern students of KUSMS were called to participate in the study. After the consent from the participants, focus groups of 6 participants (2 participants from each 3rd year, 4th year, and intern) were formed. In five FGDs, data saturation was observed and no further FGDs were conducted. FGDs were collected with audio tape recordings. The data were transcribed verbatim and translated into the English language. Then data coding was done and themes and sub-themes were generated from the codes.Results: Through the transcribed and translated data, seven themes were generated which were clinical education, student expectation, student attitude and behavior, roles and responsibilities of clinical educators, exposure, resources, and miscellaneous. These headings explained the perception of facilitators and barriers.Conclusion: The facilitators and barriers to physiotherapy clinical education were identified through student’s perspectives. These findings should be considered while preparing the guiding document for clinical education to enhance the quality of education.

Efficiency of Artificial Intelligence for Interpretation of Chest Radiograms in the Republic of Tajikistan

The article presents data from recent publications and own data on screening studies with interpretation of chest radiographs using artificial intelligence CAD (Computer-Assisted Diagnosis), which, according to WHO recommendations, provides more accurate clinical thresholds for deciding who needs to take a sputum test. Another aspect of the WHO recommendations is the cost-effectiveness of CAD as a tool for triaging patients with tuberculosis symptoms in low-income countries with a high incidence of tuberculosis. Compared with smear microscopy and GeneXpert, without preliminary sorting, the use of mobile digital X-ray machines equipped with a CAD tool reduces costs, allowing sorting of individuals suspected of having tuberculosis for testing on GeneXpert, while reducing the time to start tuberculosis treatment.Thus, conducting a study using portable X-ray machines using a CAD program is a low-cost and easy-to-implement method, does not require large funds, does not require separate rooms, is highly effective, has good image quality, allows you to quickly clarify individuals suspected of having tuberculosis, differentiating it from other pathological changes in the lungs.Our experience shows that machine analysis of chest computed tomography data, due to the higher resolution capabilities of the method and the absence of fundamental disadvantages of radiography, including the effect of shadow summation, the presence of “blind” zones, etc., is finding increasing application in both diagnostics and screening of respiratory diseases. Our use of this tool allowed us to identify additional new cases of phthisio-onco-pulmonary diseases in field conditions.

Analysis of Psychological and Physiological Responses to Snoezelen Multisensory Stimulation

This study evaluates the effects of two months of regular snoezelen therapy on anger-hostility (AH), assessing both psychological and physiological indicators before and after the intervention. Using the Profile of Mood States (POMS-2), the research measured changes in anger-hostility levels and stress responses, comparing pre- and post-intervention data. At baseline, participants exhibited relatively high levels of anger-hostility, with psychological scores averaging 40.14 (± 2.46) and physiological scores averaging 42.25 (± 2.71), indicating significant irritability, aggression, and elevated stress responses. After two months of multisensory stimulation in a snoezelen room, participants’ psychological scores decreased to 37.40 (± 2.26) and physiological scores to 39.50 (± 2.01), suggesting improvements in mood and physiological stress. The statistical analysis yielded a p - value of 0.047, indicating that the reductions were significant. These findings suggest that snoezelen therapy can be effective in reducing anger-hostility and associated physiological stress, offering a promising intervention for emotional regulation and stress management.

At present, the most commonly used surgical method is the posterior median approach, the extent and degree of neurolysis are often insufficient, so the effect of neurolysis is not well, and the functional recovery of patients after operation is often incomplete.Clinical presentation: The patient was a 17-year-old male who accidentally fell from a height and landed on his hip. The main clinical feature of the patient was persistent radiating pain in the right lower extremity with right lower limb sensorimotor disorder. The results of the X-ray examination indicated a sacral fracture and a right pubic fracture. After the injury, the patient underwent pelvic internal fixation surgery within 72 hours. Then 6 months after the surgery, there was no significant improvement in right lower limb function, and the patient came to our hospital seeking treatment. Considering the severe lumbosacral plexus injury and the history of surgery, we performed an “Anterior surgery approach combined with posterior approach for lumbosacral neurolysis” for the patient, postoperative radiation pain disappeared completely, and there were significant improvements in the muscle strength of some muscles and sensory function.Conclusion: The relaxation of the lumbosacral plexus is usually performed through a single surgical approach, which has great limitations in the effect of relaxation. Here, we demonstrate a case in which posterior lumbar incision and anterior laparoscopic lumbosacral plexus neurolysis can benefit the patient, the lumbosacral nerve was released to a great extent. We aim to bring this case to the attention of our worldwide neurosurgical colleagues and share our surgical approach to assist those who may encounter this case in the future.

Impact of Intravenous Busulfan Pharmacokinetics on Safety in Pediatric Patients who have undergone Hematopoietic Stem Cell Transplant

Introduction: Busulfan (Bu)-based regimens are crucial for myeloablative conditioning in pediatric allogeneic stem cell transplantation. Despite its efficacy, Intravenous Bu has a narrow therapeutic index and variable pharmacodynamics especially in children, heightening the risk of adverse events. This study explores Bu dosing and related organ toxicities in pediatric patients at a tertiary center in Saudi Arabia.Methodology: This retrospective study at King Fahad Specialist Hospital in Dammam (KFSH-D), Saudi Arabia, included pediatric patients (≤16 years) treated with intravenous Bu before bone marrow transplantation from 2010 to 2022. Pharmacokinetic dose adjustments were based on AUC targets of 900-1350 µMol-min. Descriptive measures included mean, Standard Deviation (SD), median, minimum-maximum values, counts, and percentages. Statistical analyses used Kruskal-Wallis, Chi-square, and Fisher’s exact tests. Ethical approval was obtained from KFSH-D.Results: We identified 44 pediatric patients who underwent Bu prior to HSCT. Mean age was 4.95 ± 2.49 years, with a female majority (56.8%). Primary diseases included Beta Thalassemia (34.09%), Neuroblastoma (29.55%) among others. There was no significant difference in the cohort’s demographic and clinical features of the cohort. Nonetheless, higher infections were found in the Low-AUC group (66.7%) compared to the Target-AUC (40.0%) and Higher-AUC groups (0.0%) (p = 0.015).Conclusion: This study emphasizes the need for therapeutic drug monitoring and individualized Bu dosing in pediatric HSCT to minimize toxicity and improve outcomes. Larger multicenter studies are recommended to refine dosing strategies and enhance the safety and efficacy of Bu-based regimens.

Congenital Dysfibrinogenaemia: A Family Case Report

Congenital fibrinogen qualitative disorders, including dysfibrinogenemia and hypo-dysfibrinogenaemia, are highly heterogeneous, both in clinical manifestation and for the mutational molecular spectrum driving these disorders. Correlations between phenotype and genotype remain poorly defined. Considerable work lies ahead in order to achieve diagnostic and prognostic precision and subsequently provide targeted management for this rare disease. Here we report the laboratory test, the clinical and molecular characterisation of a family with dysfibrinogenemia.

Sexual Dimorphism in the Length of the Corpus Callosum in Cadaver

Context: Several texts and literature suggest that corpus callosum may be sexually dimorphic. Previous researchers found that the length of the corpus callosum is larger in males than in females. Reviewing various foreign literature found that the length of the corpus callosum may change in many diseases in Neurology, Neurosurgery, and Psychiatry. So, knowledge of the normal morphological difference of the length of corpus callosum between Bangladeshi males and females is essential for the diagnosis in brain imaging and treatment of those diseases. Objective: The present study was conducted to provide data on the length of the corpus callosum of our people, which can be used to set a standard measurement for the Bangladeshi population. Materials and methods: A cross-sectional, descriptive study was done in the Department of Anatomy, Dhaka Medical College, Dhaka, Bangladesh, from July 2009 to June 2010, based on the collection of 60 human brains (male 36 and female 24) from unclaimed dead bodies. The lengths were measured by using digital slide calipers in mm.Results: The mean length of the corpus callosum in males and females in groups A, B, C & D (grouping in done on age difference) were 68.04 ± 0.99 and 67.03 ± 0.05 mm, 67.50 ± 0.13 and 67.02 ± 0.03 mm and 67.51 ± 0.03 and 67.02 ± 0.03 mm respectively.Conclusion: Statistically significant differences were found between males and females in all age groups in the length of the corpus callosum.
