Oral Suspension as Versatile Galenic Formulation in Pediatry

In the last years, there has been an increase in the prescription of drugs in pediatry as a pharmaceutical form of oral suspension. The same is true in commerce there are various producers that provide specific ready-for-use excipients to make more easier to prepare OS in the galenic laboratory. The aim of this work is to verify the advantages of this pharmaceutical form to cover pediatric dosages vs. other forms and also to overcome shortcomings of some crucial registered drugs. In this work scientific literature is reported that also relates to some ready-for-use products as bases-vehicle for suspension and some formulations of interest.

Industrial control systems: The biggest cyber threat

Industrial control systems (ICS) are critical, as in these systems, cyber threats have the potential to affect, disorganize, change their mode of operation, act as an information extraction vehicle, and ultimately turn against itself. Creating risks to the system itself, infrastructure, downtime, leakage of sensitive data, and even loss of human life. Industrial control systems (ICS) are vital to the operation of all the modern automated infrastructure in the western world, such as power plant and power stations. Industrial control systems (ICS) differ from the traditional information systems and infrastructures of organizations and companies, a standard cyber security strategy cannot be implemented but part of it adapting to the real facts and needs of each country, legislation and infrastructure. These systems require continuous operation, reliability and rapid recovery when attacked electronically with automated control, isolation and attack management processes. Incorrect settings and lack of strategic planning can lead to unprotected operation of critical installations, as they do not meet the cyber security requirements. Industrial control systems (ICS) require special protection in their networks, as they should be considered vulnerable in all their areas, they need protection from cyber attacks against ICS, SCADA servers, workstations, PLC automations, etc. Security policies to be implemented should provide protection against cyber threats, and systems recovery without affecting the operation and reliability of operating processes. Security policies such as security assessment, smart reporting, vulnerability and threat simulation, integrity control analysis, apply security policy to shared systems, intrusion detection and prevention, and finally firewall with integrated antivirus and sandbox services should be considered essential entities.

Evaluation of the Social Return on Investment (SROI) in Patients with Hepatitis C. The Case of the Mobile Outreach Program (MOP)

The World Drug Report highlights that the prevalence of hepatitis C virus (HCV) is one of the leading causes of cirrhosis and hepatocellular carcinoma globally. This report estimates that HCV is responsible for the annual death of around 700,000 people due to liver disease and is associated with high levels of morbidity and mortality with high health and socio-economic costs. This exploratory study aims to evaluate the socio-economic return provided by HCV treatment of individuals integrated into the MOP developed by the NGO Ares do Pinhal in Portugal. Using the SROI (Social Return on Investment) methodology is estimated that for every euro invested in this program, the return for society is 3,75€. This is a significantly positive SROI value, highlighting the importance of this investment in the creation of monetized value for society, as well as to provide greater well-being to those who need it and to society as a whole.

Best and effective practices of wound care and healing among patients with Diabetes Mellitus

Aim: With the continuing trend in the incidence of diabetic ulcer, the current researches on wound care and healing will bring best and effective practices on wound care and healing among patients suffering from diabetes mellitus. Methods: This study utilized a systematic review of literatures in light of wound care through Pub Med database with the key words “Diabetes Mellitus,” “wound practices,” “wound healing,” and “systematic review.” The searched literatures were of case reports, interventional studies, and review papers whose main texts were in English. Results: There were 7,988 published articles yielded from the keywords used and 16 were included in this study. The practiced management and techniques in these literatures focused on comfortable and cost efficient means of wound care and healing with the involvement of group participation than individual plan of care. Conclusions: There are meager evidences of feasible wound and healing options that needs further studies in resolving wound issues among patients with Diabetes Mellitus. This systematic review establishes the insufficiency of high level of evidence based studies on wound healing in DM patients and brings a track for continuity of rigid studies concerning this topic.

Assessment of Nutritional Knowledge, Alcohol and Tobacco Consumption in Amateur and Elite Youth Football Players

Introduction: Nutrition is increasingly recognized as a key component of optimal sports performance. On the other hand, insufficient and continuous food intake can compromise athletic performance and success where Nutritional Knowledge can play a leading role.Methods: One hundred and three youth athletes from 6 different clubs belonging to the District, Honor, and National Championship participated in this study. To assess Nutritional Knowledge, consumption of alcohol and tobacco the Abridged Nutrition for Sport Knowledge Questionnaire was applied.Results: The percentage of Nutritional Knowledge obtained in the District Championship was 31.25%, in the Honors it was 35.56% and in the National Championship it was 35.02%. No significant differences were found between the Nutritional Knowledge of athletes. In turn, there were statistical differences in the consumption of alcohol and tobacco among the athletes of the 3 Championships, with the District division having the highest percentage of smokers (100%) and alcohol consumers (42.6%).Conclusion: It is essential to increase the Nutritional Knowledge observed in these young athletes since it can have an influential role in their food choices.

Comparative Analysis of Demographic and Clinical Profiles of Conventional Retinopathy of Prematurity with Aggressive Posterior Retinopathy of Prematurity

Aim: To compare the demographic and clinical profile of conventional retinopathy of prematurity (ROP) with aggressive posterior retinopathy of prematurity.Material and methods: A prospective, unmasked, and observational study involving 150 ROP patients was conducted over a period of one year. The risk factors including maternal and neonatal risk factors were recorded. The data was entered into the Excel sheet and analyzed to compare the risk factors between the conventional ROP and APROP.Results: A total of 17 (11.3%) babies were found to have AP-ROP and 133 (88.7%) babies were found to have conventional ROP. There was no significant difference in average gestational age between infants diagnosed with AP-ROP (29.6 ± 2.9 weeks) and those with Conventional ROP (30.1 ± 2.2 weeks) (p = 0.428). However, infants with AP-ROP had a significantly lower average birth weight (1022.7 ± 123.5 g) compared to infants with Conventional ROP (1208.2 ± 261.0 g) (p  = 0.004). Multivariate logistic regression analysis revealed that birth weight, surfactant use, number of days of oxygen supplementation, and metabolic acidosis were independently associated with the development of AP-ROP.Conclusion: The development of APROP is multifactorial and complex. Although we have identified factors such as birth weight, surfactant use, number of days of oxygen supplementation, and metabolic acidosis in the causation of APROP, further long-term multicentric studies are required for validation.

Estimation of Serum Beta 2- microglobulin among newly diagnosed children with cancer in Basra

Background: Beta 2- micro globulin (β2-MG) is involved in human malignancies. Increased synthesis and release of β2-MG, as indicated by elevated serum, plasma, or urine β2-MG concentration, occurs in several malignant diseases. Objective: The study was designed to assess the role of serum Beta2- micro globulin in the support of the diagnosis of different types of pediatric malignancies. Subjects and Methods: This case - control study was carried out on 137 children and adolescents with newly diagnosed pre-treated malignant diseases who were admitted to pediatric oncology center at Basra Children’s Specialty Hospital, their ages ranged from 3 months to 15 years, during the period from the 1st of November 2014 till the end of October 2015, 71 were males and 66 were females and 148 healthy children and adolescents (83 were males and 65 were females) matched for age and sex regarded as control group. Cases and control characteristics were assessed from data collection by special questionnaire. All patients and control group were investigated for Beta2- microglobulin by the enzyme-linked immunosorbent assay. Results: The study had revealed that level of Beta2-microglobulin was significantly higher in patients with malignancy in comparison to control group, P value < 0.001.Also the serum Beta2- microglobulin level for both hematological malignancies and solid malignancies was assessed and it was found that significantly higher percentage of elevated serum Beta2- microglobulin level was present in patients with hematological malignancies in comparison to solid malignancies, P value <0.01.The study also had revealed that there was a significant correlation between the initial white blood cells count ≥ 50000 cells/ml and abnormal serum Beta2- microglobulin level, P value < 0.01,but there was no significant differences in serum Beta2- microglobulin level in relation to risk groups and immunophynotypes of acute lymphoblastic leukemia ,morphological subtypes of acute myloid leukemia, stages of each type of lymphoma (Hodgkin lymphoma and non-Hodgkin lymphoma) and the histopathological subtypes of non-Hodgkin lymphomas. After subjecting variables (specific to acute lymphoblastic leukemia) to logistic regression analysis, the significant independent risk factor that associated with abnormal serum Beta2- microglobulin level was high initial white blood cells count (≥50000 cells/ml). Conclusion: Serum Beta2- microglobulin level is significantly higher in patients with hematological malignancies and high initial white blood cells count(≥50000cells/ml) .From this study, serum Beta2- microglobulin could be recommended in the initial work up for diagnosis of childhood malignancy.

Drug Abuse and Mental Illness in Erigavo Mental Hostiptal, Erigavo, Somalia

Mental illness is an international health crisis that puts the lives of over three hundred million people (Four percent of the world’s population) at risk. The study was conducted at the Erigavo Mental Hospital in Somaliland and its main focus was the bond between substance abuse and mental illness. The research was to check the influence of the different drugs, screen mental health problems, and draw a link between drugs and mental illness. The sources of the quantitative data were a total of 27 employees and managers. This study is cross-sectional. We employed the measure of the magnitude and direction of a linear relationship between two variables with the Pearson correlation coefficient. The outcomes that were acquired show that there was use and particularly abuse of khat, substance smoking, and alcohol consumption, the score means of which were quite high. The investigation results, however, imply that through drug use and other forms of alcohol drug abuse, one of them counter various mental health problems. Furthermore, the addicted people demonstrated the tendency to use force in their attempt to solve the conflicts and to experience the highest degree of tension, with the mean score of the former giving 4.11 while the standard deviation of the latter giving 1.086, thus this shows the mental impact of drug use on the inhabitants of Erigavo. The main result of the study was the strong, positive relationship between drug addiction and mental illness, which became evident through an r - value of 0.509 and a p - value of 0.559. It means that those people who are involved in drug misuse are the ones who are likely to have mental health disorders. Based on the findings of the study, we suggest a combination of measures to recover drug addiction and mental illness. This consists in strengthening prevention. The study that lays the groundwork for suggestions, involves an approach that is holistic in nature. The main ideas of preventing drug usage, psychological health care, enhancing mental health services, and working towards certified professionals are key integrated activities of the whole concept. The hospital, thus, will be in a better position to eliminate the two major disease components namely; drug abuse and mental ailment. Thus, in this way, with the help of the new treatment approaches, a hospital provides and their patients who are quite likely to recover and lead healthy.

Radiological assessment of exposure doses and radon exhalation rates of building materials in Saudi Arabia

In the present work, samples of building material are analyzed for their naturally occurring radioisotope activity such as uranium, radium, and radon. The radon emission rates, and the annual effective doses, "AED”, are also investigated. The activity of twenty-four samples, taken from the local markets of Saudi Arabia, was determined using the "Sealed-cup Technique” and Solid State Nuclear Track Detectors, "SSNTD.” The uranium concentration activity of the samples is found to vary from 0.62 to 4.68 ppm with an average of 1.92±0.42 ppm. The radium concentration varies from 0.61 to 4.64 Bq·kg−1, with an average of 1.91±0.42 Bq·kg−1, the radon concentration in the samples varies from 42.29 to 319.97 Bq·m−3 with an average of 131.53±28.94 Bq·m−3. The value of the dissolved radon concentration in the collected samples varies from 12.99 to 98.97 Bq·m−3 with an average of 40.41±8.89 Bq·m−3. The mass exhalation rates are found to vary from 1.54 to 11.65 mBq·kg−1·h−1, with an average of 4.79±1.05 mBq·kg−1·h−1, while the surface inhalation rates vary from 76.97 to 582.35 mBq·m−2·h−1, with an average of 239.38±52.66 mBq·m−2·h−1. The AED due to indoor uses varies from 1.07 to 8.07 mSv·y−1, with an average of 3.32±0.73 mSv·y−1. The AED due to indoor plus outdoor uses varies from 1.47 to 11.10 mSv·y−1, with an average of 4.56±1.0 mSv·y−1. The results of this study show that the values obtained for most samples are within the internationally accepted recommended values. Therefore, these samples can be used as building materials as they do not pose a major risk to humans.

The Neppe-Close triadic dimensional vortical paradigm: An invited summary

Physicists are generally trained in the Standard Model of Physics (SMP). This implies that they perceive and account for only 3 dimensions of space in a moment in time (3S-1t) (a 4-dimensional [4D] model). However, applying the SMP, more than fifty significant conundrums have arisen that are unexplained or incomplete. Explaining these within the SMP 4D fabric led to hypothesizing a ‘fifth force’, most recently the hypothetical ‘X17 particle’. We propose this hypothetical X17 may better be explained by a 9-dimensional model (9D) with gimmel. Our model, the Neppe-Close Triadic Dimensional Vortical Paradigm (TDVP) has amplified the ‘physics’ from 4 dimensions to 9D, specifically first postulating and then further demonstrating mathematically—starting with derivations of the Cabibbo angle—that 9 dimensions must exist. Moreover, this data is empirically demonstrated because the neutron, proton and electron mass-energy-gimmel equivalence in the Triadic Rotational Units of Equivalence (TRUE) as part of the TDVP model, exactly corresponds with the normalized data for the mass-energy equivalence volumetric data for these particles in the CERN Large Hadron Collider. This data shows definitively that we exist in a 9-dimensional finite, quantized, volumetric, spinning reality. This is, furthermore, embedded in an infinite continuity (9D+). Mathematically, applying this 9D+ model definitively requires an extra third component that is massless and energyless (‘gimmel’). Without gimmel, no particle in the universe would be stable. TDVP unifies nature because the same laws apply across the quantum, macro-world and cosmological reality. Our 4D experience is simply the physical component of 9D+ existence. Summary Amplification: At all levels, there is the consistent application of a 9-Dimensional quantized finite reality embedded within an infinite continuity. The application of gimmel specifically requires applying the 9-dimensional model and is based on necessary mathematical calculations not only at the quantal level (where the fifty plus unsolved, unexplained or contradictory conundrums can be explained somewhat, and there is no longer ‘quantum weirdness’), but at the macroscale level with more gimmel in the life elements (which, additionally, are consistently all cubic multiples of 108 cubed), as well as cosmologically, where the correlations with proportionate Dark Matter and Dark Energy are overwhelming. Moreover, these 9-dimensional plus factors together with Triadic Rotational Units of Equivalence (TRUE) and gimmel, allow numerous solutions that couldn’t otherwise be solved. For example, importantly, applying the simple mathematics of TRUE, we can demonstrate why gluons, while adequate in 4D, are impossible applying 9D. These solutions are simpler because we have markedly adapted George Spencer-Brown’s ‘Laws of Form’ to applying a new method of mathematical calculation, Edward Close’s ‘Calculus of Distinctions’ (COD) which recognizes quantal limits and that the nature of finite reality is quantized and volumetric. The COD includes distinguishing between content, extent, and impact. We emphasize the pioneering works of Wolfgang Pauli with his multidimensional model and his ‘Pauli Exclusion Principle’, Alfred Whitehead with ‘Process Philosophy’ and his ‘Principia Mathematica’ (with Bertrand Russell), Georg Cantor with Set Theory, and Roger Penrose with spinors and twistors. TDVP is a prime example of our broad new specialty of ‘Dimensional Biopsychophysics’ (DBP). DBP extends physics, consciousness, and the biopsychosocial to extra dimensions and applies mathematics empirically. Like Max Tegmark, we recognize the key role of mathematics as fundamental in nature, not just for application in calculation and operations.
