Ivabradine versus carvedilol in the management of palpitation with sinus tachycardia among recovered COVID-19 patients

Introduction: One of the major complications among COVID-19 patients include cardiac arrhythmias. Commonest arrhythmia is sinus tachycardia which is usually associated with palpitation causing discomfort to patients. In this study, we present a comparative study of use of Ivabradine vs. Carvedilol for sinus tachycardia in post-COVID-19 infected patients. Method: 50 consecutive recovered COVID-19 patients with sinus tachycardia were included in this open labelled RCT. 25 patients received Ivabradine and remaining 25 received Carvedilol. Single therapy non-responders were treated with Ivabradine with Atorvastatin. Results: The mean age of all patients is 48.8±7.66 years (Males 49.5 ± 7.21 years; Females 47.68 ± 8.23 years). The mean heart rate (MHR) of all patients is 125.52 ± 9.07/min (Males 125.67 ± 8.78/min; Females 125.26 ± 9.5/min). After five days of single drug therapy the mean drop in the heart rate was 35.04 ± 10.55/min (Males 34.41 ± 9.71/min; Females 36.05 ± 11.72/min), resulting in 27.88 ± 8.11% (Males 27.38 ± 7.56%; Females 28.69 ± 8.89%) reduction in MHR. Among the two groups, the Carvedilol group showed improvement of MHR in 14(56%) patients; whereas in Ivabradine group 18(72%) patients improved out of 25 patients each (p: 0.2385). In the Carvedilol group the MHR reduced from 128.6 ± 8.44 to 95.68 ± 10.63 (p < 0.001), which is statistically significant; similarly, the Ivabradine group showed a MHR from 122.44 ± 8.62 to 85.28 ± 10.52 (p < 0.001). The monotherapy therapy non-responders were treated with dual-therapy of (Ivabradine + Atorvastatin). Discussion: Ivabradine is more effective in controlling heart rate compared to Carvedilol. Also, Ivabradine group scores very well in ‘patient-satisfaction’ with regards to symptom (palpitation) relief. Conclusion: The COVID-19 sequelae of sinus tachycardia can be better controlled with Ivabradine when compared to Carvedilol.

Prevalence of disabling hearing loss in the elderly

Introduction: Disabling hearing loss refers to hearing thresholds superior than 40 dB in the better ear in the adults. The main cause of hearing loss in the elderly is the age-related hearing loss, also called presbycusis. This type of hearing impairment occurs as individuals grow older and is usually sensorineural hearing disorder greater for high-pitched sounds and affects both ears equally. It is estimated that 466 million people worldwide have disabling hearing loss, one third of which are over 65 years old. Objective: To analyze the prevalence of disabling hearing loss in the elderly of Juiz de Fora. Methods: Cross-sectional study with 122 patients. Pure tone audiometry was performed after meticulous physical examination of the external ear. Results: Out of 122 older adults, 85 (69,6%) presented disabling hearing loss. Conclusion: Hearing loss, specially disabling hearing loss, is a frequent condition in the elderly and has a big impact on their quality of life. For that it should be promptly diagnosed so treatment can be initiated.

Neural Network Calculator of Rubber Characteristics with Improved Properties

A new technique for the use of Artificial Neural Networks (ANN) for the generalization and visual presentation of the results of experimental studies is proposed. The possibility of using ANN for cases for which their use was previously considered impossible is shown. ANN calculators have been created that summarize the results of experimental studies on the effect of trans-polynorbornene and basalt fiber on the characteristics of a rubber compound based on general-purpose rubbers (isoprene SKI-3, butadiene-methylstyrene SKMS-30ARK and butadiene SKD), which also contained vulcanizing agents (N, N′-dithiodimorpholine, thiuram D), vulcanization accelerators (sulfenamide C, 2-mercapto-benzothiazole), vulcanization activators (zinc white, stearic acid), emollients (industrial oil I-12A, rosin) and antioxidants (acetonanil H, diaphene FP). The rubber mixture was prepared on laboratory rollers LB 320 160/160. Subsequently, the rubber mixture was vulcanized in a P-V-100-3RT-2-PCD press. For the resulting vulcanizates, the physical and mechanical properties and their changes were determined after daily exposure to air and in a standard SZhR-1 hydrocarbon liquid at a temperature of 100 °C. We also studied the change in the mass of vulcanizates after exposure to industrial oil I-20A and water. The dynamic parameters (modulus of elasticity and mechanical loss tangent) of vulcanizates, which characterize their noise and vibration-absorbing properties, were studied on a Metravib VHF 104 dynamic mechanical analyzer. The created ANN calculators allow solving a direct problem - interpolating the dependences of all rubber characteristics on the content of basalt fiber, as well as solving inverse problems - to determine the required content of basalt fiber to create rubber with the required performance properties. The autonomous executable modules of the calculators developed by ANN were made and can be passed to everyone.

Water Purification Using Ceramic Pots Water Filter

In this study, ceramic pot filters are made from clay and burn-out materials (sawdust) that give pore sizes capable of capturing contaminants. Manufacturing specifications were selected to achieve some results. Clay and sawdust are mixed in a 50% volume ratio each and sawdust was subjected to hot water extraction to give a treated sample. Filters produced comprised of untreated, treated, and a mixture of treated and untreated sawdust samples, some of which were dipped in a solution of silver nanoparticles while others were not dipped (treated undipped, treated dipped, mixed dipped, mixed undipped, untreated undipped, untreated dipped). The effectiveness of the produced filters for the removal of contaminants such as dissolved solids, turbidity, and metals was tested using water collected from the Ikeji Arakeji River in Osun, Nigeria. The results showed the filter with treated sawdust undipped in a solution of silver nano gave the best result in the removal of the contaminants. Also, the filter with the mixtures of treated and untreated sawdust gave a better result compared to the standard. While the standard gave a better result than the untreated undipped ceramic filter pot. In conclusion, with proper cleaning and maintenance of the filters, they can effectively provide treated water suitable for drinking to rural people affected by polluted water sources.

Importance of the radiations in water splitting for hydrogen generation

The review article examines the production of molecular hydrogen from the decomposition of water by various irradiation methods. The article shows different types of radiation: UV radiation, visible radiation, gamma radiation, X-ray radiation and neutron radiation. Electrons generated by radiation inside a nanoparticle of radius R suspense in fluid water are diffused with equal probability in all directions inside the particle and gradually lose their kinetic energy as a result of elastic and inelastic collisions. Some of these electrons are transported to the nanoparticle surface during the physical and physicochemical stages of the process and emitted into the water. It is extremely important for the formation of nanostructured materials after exposure to ordered nanostructure from the new phase with a period of a few nanometers, promoting the preservation of the properties of materials under high irradiation.

Enclosure asphyxia as a cause of death in 3 Nigerian children trapped at the boot of car-Case reports and review of literature

The paper reviews enclosure asphyxia as a cause of death of three Nigerian children aged three years, five years and seven years respectively, who were found dead and locked in the boot of a Mercedes Benz 200E Sedan car a day after they were declared missing. The first two were siblings of the same parent whose fathers actually owned the car, while the older child was a neighbour kid all of whom were well known to each other as friends and playmates. They had gone missing for a few hours prior to being seen playing together. Search parties were raised and they were never found until the following day when they were found dead in the boot of the car parked a few meters away from the house under a tree. Incidentally the same car was driven round the neighbourhood for 3 hours during the search the previous day with no inkling that they were in the boot of the car. Autopsy was ordered by the coroner to determine the cause of death.

Associations of Burnout, Secondary Traumatic Stress and Individual Differences among Correctional Psychologists`

Objective: The aim of the present study was to examine associations between secondary traumatic stress (ST), job burnout (BO) and several psychological variables such as world assumptions and locus of control in correctional psychologists. Methods: This study utilized information provided by 87 currently prac¬ticing correctional mental health providers (psychologists) in the correctional settings across Russia in St.-Petersburg, Belgorod, Vladimir, Kaluga, Ryazan, etc. The sample included 51 men, 36 women. The mean age of participants was 34.9±6.9 (ranging from 25 to 48 years). Participants reported working a mean of 6.23±3.5 years (ranging from 3 months to 15 years) in a correc¬tional setting. Subjects were assessed with Maslach Burnout Inventory-Human Services Survey (MBI-HSS), Secondary Traumatic Stress Scale (STSS), World Assumption Scale (WAS) and Locus of Control (LC) Scale. Three sets of statistical analysis were provided: ANOVAs between- group comparisons with STS and BO as factors and Spearman correlation analyses. Results: The results of our study reveal that burnout and secondary traumatic stress in correctional psychologists are significantly positively related and thus may be exacerbated by each other. BO is significantly negatively associated with WAS benevolence scale and the WAS self worth scale and STS is significantly negatively associated with WAS benevolence scale and the WAS meaningfulness scale. However, LC and its components are negatively associated with BO, but not with STS. Conclusion: The main future direction of our research is to construct nonlinear model of burnout with STS, WA and LC components as predictors, identify its parameters and make its validation.

Failure to Autopsy: The Otto Warmbier Case

Coroner and medical examiner offices are charged with the medicolegal investigation of deaths to determine the cause and manner of death. We describe the recent high-profile case of Otto Warmbier, who tragically died shortly after his return in a coma from North Korea and in which the coroner failed to conduct a complete autopsy, and failed to satisfy the needs of the local, national, and international communities. Medico legal death investigation offices, including both medical examiner and coroner offices, have a legal responsibility and duty to investigate deaths in the public interest for public safety and public health purposes. Although, they serve the families of the deceased as they can, their raison d’être and priority is service to the greater public good. Thus, they may conduct investigations and even autopsies over the objections of the next-of-kin. Full investigations include a complete forensic autopsy. The utilitarian public interest should, within the constraints of the office resources, drive the decision of whether to autopsy or not. Failure to autopsy can sometimes constitute a breach of faith that the public entrusts in these offices. Empirically, this seems to be a greater problem in coroner jurisdictions than in medical examiner offices. Specifically, we believe the recent case of Otto Warmbier is such a case.

Awareness level on the relevance of forensics in criminal investigation in Nigeria

The main aim of forensic science is to gather intelligence to enable the judge to credible and logical decisions in the court by means of scientific approach through evaluation of evidence for the administration of justice, and country around the world now considers forensic methodology as the gold standard for criminal investigation. Therefore, the present study examined the level of awareness on the relevance of forensics in criminal investigation in Nigeria. The design used in this study is the survey research design and the sample size of this study was a total of one hundred personnel of law enforcement and the judiciary. The study adopted descriptive statistics which involves the use of frequency and percentage. The result of the present study revealed that the participants were distributed socio-demographically as follows; there was an observable higher number of male participants (68%) relative to the female participants (32%), As per age distribution, a larger population of the participants were found to be > 40 years of age with 55%, and it was observed that age between 35-39 years ranked the least with 15%. On educational level, the result of the present study revealed that majority of the participants possesses a bachelor’s degree as the highest level of educational qualification with 75% from a pool of 100% of participants. The present study further examined responses on the relevance of forensics in criminal investigation, and the result revealed an inadequate level of awareness on the relevance of forensics in criminal investigation. Therefore, the study recommends that the Nigerian Police Force and the Judiciary should collaborate with Universities running programs on forensics for trainings.

Genetic identification of three exhumed human remains at a hospital in Ghana: a forensic case report

DNA identification is very important in cases of high decomposition of dead bodies, in which the bodies cannot be identified by physical means.To compare the results of DNA typing, it is necessary to have related subjects with which to perform comparative analyses. Such tests are normally performed by comparing DNA profiles from people known to be immediate family members of the presumptive victim, such as parents or children because they share half of their genetic material with the unidentified.We report on how DNA analysis was used to solve a case of mixed-up bodies at a local mortuary in Ghana, West Africa. Two families and three buried human remains were in contention in this case. The first body (E9) was buried three months before exhumation. The second body (E11) was buried two and a half months before exhumation whiles the third body (E10) was buried a month before exhumation. Exhibit E5 was taken from an alleged child of the deceased, E11. Toenails of the exhumed bodies were sampled by a pathologist and used for DNA extractions using the QIAamp DNA Investigator Kit. Profiles from relatives were generated for comparison purposes. All samples gave a quality amount of genomic DNA after quantification. DNA was amplified with a GlobalFiler PCR amplification kit. Profiles from relatives were generated for comparison purposes.The human remains (exhibit E11) cannot be excluded as the biological father of the child (exhibit E5) because they share common alleles at all 23 genetic loci. The applicable combined paternity index was 17218125604.492 assuming a prior probability of 0.5. The probability of paternity is 99.99999999%. Based on this relationship testing, one of the bodies was successfully identified and handed over to the family for re-burial.
