Vaginal cervicoisthmic cerclage for cervical incompetence in pregnant women: Fernandez’s technique in 8 steps

Video objective: To demonstrate that surgical technique of vaginal cervicoisthmic cerclage must be performed in women with history of cervical incompetence with more than two late miscarriages before 24 weeks or premature deliveries before 28 weeks and after prior failure of preventive Mc Donald cerclage. Effectiveness, longevity, and color stability of in-office bleaching (6% H2O2 gel/Violet LED) and diastema closure with direct composite: 3-year follow-up

To reduce bleaching side effects, the use of low concentrations of Hydrogen Peroxide (HP) agents associated with hybrid light (violet LED/Diode Laser) has gained interest. Case report: The aim of this report is to describe a case of a 16-year-old patient that presented a complaint related to the color of his teeth and a maxillary midline diastema. In-office bleaching with 6% HP associated with hybrid light (violet LED/Diode Laser) was performed. The bleaching gel was applied once on the teeth and light-activated for 1 minute (15 times) followed by 1min intervals (15 times) with a total bleaching time of 30 minutes. After the bleaching procedure, the teeth were polished and the desensitizer was applied for 4 minutes. Two bleaching sessions were performed at a 1-week interval. The diastema was closed with direct resin composite restorations without any tooth preparation. The conventional 3-step bonding agent was used and A1 dentin shade and B1 enamel shade were used followed by polishing discs. At 3-year recall, discoloration and fractures were not found on the the teeth or restorations and patient was completely satisfied. Conclusion: the conservative and safe option of bleaching with a low-concentrated HP gel associated with violet LED light is an interesting option for young patients and presents longevity over time.

Imaging aspect of neuromyelitis optica: a case report and review of the literature

The case report presents a neuromyelitis optica in a 19 years old male. Brain and spinal cord MRI showed bilateral optic neuropathy, multiphasic demyelinating process involving the cervical and thoracic spinal cord. Cerebrospinal fluid showed negative NMO Ig G. We will describe the radiological aspect of neuromyelitis optica with a review of the literature.

A comparitive study on square stepping exercise versus balance training exercises on fear of fall and balance in parkinsonism patients

To study the effect of square stepping exercise and balance exercise to improve balance impairment and reducing the fear of falling on Parkinsonism patient. This is a pre-test and post-test experimental design comparative in nature. Subject aged between 60 – 70 years having Parkinson’s disease were included for the study. Group A (15 subjects) participated in square stepping exercise for 5 times in a week for 4 weeks and Group B (15 subjects) participated in balance exercise for 5 times in a week for 4 weeks. The outcome was measured by using Berg balance scale, Time up and go (TUG) test and fall efficacy scale. Statistical analysis done by using pair ‘t’ test and unpair ‘t’ test showed that there is significant improvement in subject who underwent the square stepping exercise program compared to balance exercise on fear of fall and balance in parkinsonism patients. It is concluded that square stepping exercise group exhibited statistically significant improvement in Berg Balance Scale, Time up and Go test score and fall efficacy test score compared to the balance training exercise group.

Neo communicable disease rather than ‘non’ communicable disease for the acronym “NCD”

It is with interest and pleasure that I see notices of meetings and symposia, and articles, devoted to the theme Noncommunicable disease. I still live in the economic and geographical ‘South’ but the concern arises from the continued use of terminology that dulls the senses to the urgency of the problems involved. Since I wrote the letter (reference at end) the epidemic of the so called ‘non’ communicable disease has increased greatly; obesity, cardiovascular disease, diabetes, cancers, accidental trauma, road deaths etc, and there is still less research and action about breaking the methods of spread than there is about the illnesses.

Assessment and sensitive analysis of biological water risks in water resources with application of classical mass transfer computations

Due to the urgent need for water in all parts of industrial or developing societies, water supply, and transmission facilities are suitable targets for biological risks. Given that even a short interruption in water supply and water supply operations has a great impact on daily activities in the community, the deliberate contamination of urban water resources has irreparable consequences in the field of public health, and the economy of society will follow. Unfortunately, most officials in the public health control departments in our country have received limited training in detecting accidental or intentional contamination of water resources and dealing with the spread of waterborne diseases both naturally and intentionally. For this reason, there is low preparedness in the responsible agencies to deal with waterborne diseases during biological risks. In the first step of this research, a review study has been conducted on water biological risks and operational strategies to deal with them. In the following, it has studied how Escherichia coli (E. coli) bacteria spread in aqueous media. In this regard, the kinetic model of the studied microorganism was analyzed based on the implementation of (Fick Law) in polar coordinates and the combination of (Dirac Distribution) with (Legendre polynomial) distribution. Finally, after studying the factors affecting the microbial pollutant emission coefficient, the effects of all three factors of linear velocity, linear motion time period, and angle of motion on the pollutant emission flux and biofilm diffusion time in the water supply network environment were investigated. Studies have shown that the linear velocity parameter of Escherichia coli with a nonlinear relationship has the greatest effects on the release of microbial contaminants.

Beneficial effects of a ketogenic diet in a woman with Charcot-Marie-Tooth disease

Objective: To evaluate the effects on quality of life and body composition of a Ketogenic Diet (KD) in a woman with Charcot-Marie-Tooth (CMT) disease. Methods: Physical (PCS) and mental (MCS) health conditions were evaluated with the SF-36 questionnaire; dual-energy x-ray absorptiometry was used to determine body composition; parameters were determined at baseline and after 12 weeks of KD.Results: At baseline PCS and MCS were 20.6 and 20.7 respectively with 37.9% fat mass. After 12 weeks SF-36 values significantly improved: PCS 55 and MCS 66.1 with 33.9% fat mass. Conclusion: KD improved the patient’s quality of life and decreased fat mass. Further studies will be needed to better elucidate the beneficial effects of KD among people with CMT.

Harnessing Artificial Intelligence for Early and Evolution of Alzheimer’s Disease Detections and Enhancing Senior Mental Health through Innovative Art-Singing Therapies: A Multidisciplinary Approach

The well-documented therapeutic potential of group singing for patients living with Alzheimer’s disease (PLAD) has been hindered by COVID-19 restrictions, exacerbating loneliness and cognitive decline among seniors in residential and long-term care centers (CHSLDs). Addressing this challenge, the multidisciplinary study aims to develop a patient-oriented virtual reality (XR) interaction system facilitating group singing for mental health support during confinement and enhancing the understanding of the links between Alzheimer’s disease, social interaction, and singing. The researchers also propose to establish an early AD detection system using voice, facial, and non-invasive biometric measurements and validate the efficacy of selected intervention practices. The methodology involves co-designing an intelligent environment with caregivers to support PLAD mental health through online group singing, addressing existing constraints in CHSLDs. The researchers will engage volunteers in remote singing interactions and validate the impact of voice stimulation for PLADs using a control group. The primary expected outcome is the development of an “Intelligent Learning Health Environment,” fostering interactions while adapting to individual PLAD situations and incrementally accumulating knowledge on AD signs. This environment will facilitate the transfer of knowledge and technologies to promote non-verbal interactions via singing, enabling intervention at the first symptoms. Additionally, the research will contribute to transforming CHSLDs’ living environments, informed by neuroscience insights, and potentially extend the “collaborative self-care” approach to support seniors in aging safely and healthily at home.

Medicinal plant Potentilla fulgens and its effect in vitro against Fasciola gigantica

Fascioliasis is a one of the most important serious parasitic zoonotic disease which caused by trematode giant liver fluke Fasciola hepatica and F. gigantica among cattle’s and humans. The infection of Fasciola can be control by the use of phytochemicals as anthelmintic components. The anthelmintic activities of dried root powder of medicinal plant Potentilla fulgens and their different preparations (organic extracts and column purified fraction) are uses in vitro against liver fluke F. gigantica. The dried root powder, different organic extract, and column fractions were time and concentration-dependent. Among all the organic extracts, ethanol extract was high toxic than other organic extracts. The toxic effect of ethanolic extract of P. fulgens after 2h exposure the LC50 value is 5.22 mg/ml against F. gigantica. The column purified fraction of dried root powder of P. fulgens shows more toxicity. The 2h LC50 of column purified fraction was 3.25 mg/ml whereas in 8h exposure the LC50 is 1.24 mg/ml. The phytochemicals of the P. fulgens may be used as anthelmintic components against liver fluke F. gigantica.
