Diverticula can affect all segments of the gastrointestinal tract, from the esophagus to the colon. In order of decreasing, the jejunoileal location is the least frequent location [1] and has a prevalence of less than 2% of the population [2]. This location was first described by Sommering in 1794 [3]. More than two-thirds of small bowel diverticula occur in the jejunum. They appear mainly after the age of 60 with higher prevalence in males and rarely occur in patients under the age of 40 [4]. Jejunal diverticula are in general multiple and bigger than ileal ones [5]. Most of them are asymptomatic and do not require surgical treatment. Clinical presentations are diverse and not specific with no pathognomonic clinical symptoms.
Non-chain HF (DF) lasers are the most suitable and ecologically safe source of powerful and energetic coherent radiation in the 2.6-3.1 cm (HF laser) and 3.5-4.1 cm (DF laser) spectral regions. Among the different methods of HF (DF) pulse and pulse-periodic laser creation suggested by our team under the guidance of Academician A.M. Prokhorov was self-sustained volume discharge (SSVD). It is well known that a SSVD can be established in a gas by creating a primary electron density that exceeds a certain minimum value nmin throughout the discharge gap.
A 60-year-old female patient presented with typical anginal pain on exertion and relieved by rest for about one month. Percutaneous coronary angiography was done and showed an abnormal left circumflex coronary artery connecting to intercostal artery. Embolization of that abnormal connection was done successfully and the patient discharged from hospital after 24 hours. This case shows a new form of coronary steal syndrome. This cause could be missed if not put under the differential diagnosis of typical anginal pain with normal coronary arteries.
Dual antiplatelet therapy (DAPT) combining aspirin and a P2Y12 receptor inhibitor has been consistently shown to reduce recurrent major adverse cardiovascular events (MACE) in patients with acute coronary syndrome (ACS) or undergoing percutaneous coronary intervention (PCI) for stable coronary artery disease (CAD) compared with aspirin monotherapy but at the expense of an increased risk of significant bleeding. Among patients with stable CAD undergoing PCI with drug-eluting stents (DES), shorter duration of DAPT (3–6 months) were shown non-inferior to 12 or 24 months duration concerning MACE but reduced the rates of major bleeding? Contrariwise, prolonged DAPT durations (18–48 months) reduced the incidence of myocardial infarction and stent thrombosis, but at the cost of an increased risk of majör bleeding and all-cause mortality. Until more evidence becomes available, the choice of optimal DAPT regimen and duration for patients with CAD requires a tailored approach based on the patient clinical presentation, baseline risk profile and management strategy. Patients with acute coronary syndromes (ACS) and a history of atrial fibrillation (AF) have indications for both dual antiplatelet therapy (DAPT) and oral anticoagulation (OAC). Triple therapy (TT), the combination of DAPT and OAC, is recommended in guidelines. This article provides a contemporary state-of-the-art review of the current evidence on DAPT for secondary prevention of patients with CAD and its future perspectives.
Indonesia is the fourth largest country in the world by population and has made considerable progress since the 1960s in terms of economic growth and structural transformation. But in recent years it has become apparent that the health of the nation is far from satisfactory. The purpose of this article is to review recent literature on health problems in Indonesia and to outline the policy challenges which face the government. Since 2001, the provision of primary health care has been decentralized to sub-provincial levels of government but they lack often lack the resources, and trained staff to provide adequate services to rural populations in particular. The literature suggests that while most health indicators have shown some improvement in recent years, there are still large variations by region and social class. The country is still far from achieving the goal of universal health coverage.
The creation of the information society is associated with the creation of new intellectual, cultural, spiritual and material values, as well as with new principles and methods of social and interpersonal communication. Achieving this goal is impossible without changes in teaching methodology, teaching technologies and teacher’s work.The article is an overview and focuses on the following issues. Introduction: Dead Sea Salt, rich in minerals and ionic compositions and low in Sodium Chloride (NaCl) has many reported unique properties that set it apart from other salts. Objectives: To evaluate the composition of Dead Sea Salt and assess its in vitro cytotoxicity, and efficacy against oral bacterial leukotoxins, oral endotoxins and oral glucan sucrase. Methods: The cytotoxicity was evaluated in an established cell line (solution at 5000 µg/mL of culture medium) using positive and negative control groups. The effect on oral bacterial leukotoxin (LtxA) and different concentrations of lipopolysaccharide and glucan sucrase was established at 24, 36, 48, 60, 72, 84, and 96 hours using the HPLC method (high-performance liquid chromatography). Results: The most predominant elements detected were the water of crystallization (H2O, water that is found in the crystalline framework of salt and which is not directly bonded ), magnesium chloride (MgCl2), potassium chloride (KCl), sodium chloride (NaCl), calcium chloride (CaCl2), bromide (Br -) and sulfates (SO4). In vitro, Dead Sea Salt presented no cytotoxicity and was highly effective against leukotoxin, endotoxin, and glucan sucrase enzyme. Conclusion and clinical significance: We believe that rinsing with Dead Sea Salt has the potential to contribute to the prevention of periodontal, peri-implant and dental disease and merits clinical research.

Purpose: Exercise has a positive regulatory effect on gut microbiota and is also involved in regulating multiple physiological functions of the human body. This article tested the effects of basketball exercises at different exercise intensities on the gut microbiota of college students. Methods: Athlete research subjects (male, aged 18 - 25) were selected from the basketball team and trained at different intensities to obtain a total of 101 fresh fecal samples. DNA was extracted by a DNA extraction kit and bacterial 16S rRNA gene V3-V4 region high-throughput sequencing using the Illumina Hiseq platform. The downstream data were spliced, filtered and de-trimerized and then used to study the difference in gut microbiota. Results: Key bacterial taxa in the gut that responded to exercise intensity differed among athletes of different exercise intensities but most belonged to Firmicutes. With increasing exercise intensity, Butyricicoccus, Anaerostipes, Oxalobacter and Clostridium_IV in basketball players enrich. Further analysis of the functional prediction revealed that carbohydrate metabolism, amino acid metabolism, metabolism of xenobiotics and glycans and metabolism were significantly expressed in the gut microbiota of basketball athletes with high intensity. Conclusion: The study demonstrated that after long-term professional training, the gut microbiota of athletes adapts to exercise stimulators and can quickly respond to changes in exercise intensity. In high-intensity training, the organism is protected from harm by enriching some beneficial bacteria.
A 3-year-old non-lactating pet goat was referred to our clinic due to advanced ocular lesions and blindness of the left eye (Figure 1). According to the case history, two weeks ago, a grass awn penetrated and injured the eye. The awn was removed by the owner immediately. The following day, the goat had serous ocular discharge and photophobia and was referred to a private veterinarian. The veterinarian did not find any remaining piece of the awn and prescribed tetracaine eye drops to be administered twice a day for the next 4 days. The treatment was not successful and the eye’s condition deteriorated the following days.

Parkinson’s disease is a progressive and debilitating neurodegenerative disorder affecting millions of people worldwide. The disease is characterized by motor symptoms such as tremors, rigidity and postural instability, as well as non-motor symptoms such as depression and cognitive impairment. While there is no cure for Parkinson’s disease, there are various treatments available to manage symptoms and improve quality of life for patients.This case study examines a 65-year-old retired accountant, Mr. John Smith, who was diagnosed with Parkinson’s disease five years ago. Mr. Smith has been treated with a combination of medications, including levodopa and carbidopa and physical therapy to manage his symptoms. However, his symptoms have not significantly improved.This literature review explores the current research on Parkinson’s disease, including its pathophysiology, diagnosis and treatment. Parkinson’s disease is caused by the degeneration of dopamine-producing neurons in the brain, leading to a depletion of dopamine and the accumulation of alpha-synuclein protein, oxidative stress and inflammation. Diagnosis is based on clinical symptoms, neurological examination and response to dopaminergic therapy. Treatment focuses on managing symptoms, with medications and non-pharmacological interventions such as exercise and physical therapy. Deep brain stimulation is a surgical treatment option that has been shown to be effective in managing motor symptoms.While there is currently no cure for Parkinson’s disease, ongoing research into its pathophysiology and treatment holds promise for improving outcomes for patients. This case study highlights the importance of early diagnosis and personalized treatment plans for patients with Parkinson’s disease.
